2. Be ready to cover the cost (typically $85-95 for this service) via cash or credit/debit card.

3. You may go to any of the listed facilities in order to undergo the livescan process:

Most UPS stores (make sure to call beforehand in case an appointment is needed)

Most local police departments (make sure to call beforehand in case an appointment is needed)

Businesses and facilities listed on the Office of the Attorney General Website

Have the facility print you a copy of the completed form if possible

4. Submit completed form to volunteer@sfcityimpact.com
